The Voter Registration Form (GAB-131) must be completed accurately and completely. The form should be mailed to: City Clerk, 2000 N. Calhoun Road, Brookfield, WI 53005. Registration is NOT permitted on Saturday, Sunday, or Monday before the Election. You may register at one of the following locations or online:
You may register online if we are in the open registration period (more than 20 days from Election Day) and your driver's license contains your current name and address.  Other methods to register include by mail or in-person at the City Clerk’s Office, Brookfield Public Library during regular business hours, or at the polls on Election Day.  Be aware that there are time restrictions that registration by mail or online is accepted. The application must be entered online or postmarked (if mailing) not later than the 20th day (third Wednesday) before an election.

If you have moved from the City of Brookfield to another municipality, you may fill out the Authorization to Cancel Registration (PDF) form. Upon receiving the form, the Clerk's office will then cancel your voter registration.

Application for Absentee Ballot
If you are not already registered, you will need to register to vote before an absentee ballot can be sent to you. All requests for an Absentee Ballot must be in writing by the elector. Photo ID is required with the application. For more information see further information detailed on the back of the application for absentee ballot.
Request by Mail or Online
Complete the Application for Absentee Ballot form and mail it to the City Clerk's Office at 2000 N. Calhoun Road, Brookfield, WI 53005 or click on this link: MyVote Site to go online and request a ballot.   The application must be received by the clerk no later than 5:00 PM on the Thursday prior to Election Day in order for an absentee ballot to be mailed to you.

Request in Person
You can request an Application for Absentee Ballot in person at the City Clerk’s Office. This can be done beginning the third Monday before the Election and ends at 5:00 PM on the Friday before Election Day.

Deadline for Returning Ballot
The completed ballot must be received by the City Clerk’s Office no later than Election Day in sufficient time to deliver it to the polling site for counting. A ballot may be personally delivered directly to the voter's polling site by the elector or elector's designee prior to the 8:00 pm., closing of the polls.

Application for Poll Worker Appointment
