Voter Registration Form
The Voter Registration Form (GAB-131) must be completed accurately and completely. The form should be mailed to: City Clerk, 2000 N. Calhoun Road, Brookfield, WI 53005. Registration is NOT permitted on Saturday, Sunday, or Monday before the Election. You may register at one of the following locations:
You may register by mail or in-person at the City Clerk’s Office, Brookfield Public Library during regular business hours, or at the polls on Election Day. Be aware that there may be time restrictions that registration by mail is accepted. The application must be postmarked not later than the 20th day (third Wednesday) before an election.
If you have moved from the City of Brookfield to another municipality, you may fill out the Authorization to Cancel Registration form. Upon receiving the form, the Clerk's office will then cancel your voter registration.
Application for Absentee Ballot
If you are not already registered, you will need to register to vote before an absentee ballot can be sent to you. All requests for an Absentee Ballot must be in writing by the elector.
Request by Mail
Complete the Application for Absentee Ballot form and mail it to the City Clerk's Office at 2000 N. Calhoun Road, Brookfield, WI 53005. The application must be received by the clerk no later than 5:00 PM on the Thursday before the election in order for an absentee ballot to be mailed to you.
Request in Person
You can request an Application for Absentee Ballot in person at the City Clerk’s Office. This can be done beginning the third Monday before the Election and ends at 5:00 PM on the Friday before the election.
Deadline for Returning Ballot
The completed ballot must be received by the City Clerk’s Office no later than 4:00 p.m. on the Friday after the election as long as it is postmarked on the day of the Election. A ballot may be delivered directly to a polling place by an elector prior to 8:00 p.m.
